What Medicaid patients should know
EPSDT Referral Required (Ages 20 & Younger)
Alabama Medicaid patients ages 20 and younger are required to have a referral from their pediatrician or primary care provider before their initial appointment. Referrals can be submitted by:

Please complete all intake forms prior to your appointment to maximize your time with your provider.
Cancellation & No-Show Policy
We kindly ask for at least 24 hours' notice to cancel or reschedule. Late cancellations and missed appointments may result in a $50 fee, which insurance does not cover. Repeated missed appointments may affect future scheduling.
