MENTAL HEALTH SCREENING

Brief screenings, done securely

As part of your intake, we use brief, evidence-based screening tools to better understand how you've been feeling — so your first appointment starts with real insight, not a blank page.

Depression Screening

The PHQ-9, a brief 9-question check-in about mood, energy, and daily functioning.

Anxiety Screening

The GAD-7, a short 7-question screening for worry, restlessness, and tension.

Safety Screening

The Columbia-Suicide Severity Rating Scale (C-SSRS), reviewed with care by our clinical team.

Your privacy is protected

Because screening responses contain protected health information, all screenings are completed through our secure, HIPAA-compliant intake system — never through an unsecured website form. Your responses flow directly to your clinical team and are reviewed before your appointment.

Please complete all intake forms prior to your appointment to maximize your time with your provider.

Medicaid Appointments

Cancellation & No-Show Policy

We kindly ask for at least 24 hours' notice to cancel or reschedule. Late cancellations and missed appointments may result in a $50 fee (not covered by insurance), and repeated missed appointments may affect future scheduling.
