A first psychiatric evaluation can feel like a lot. Most families arrive carrying weeks or months of worry — behavior they could not explain, school concerns that kept coming up, or emotional patterns that did not respond to anything they tried. The evaluation is where that starts to make sense.

What a psychiatric evaluation is — and what it is not

A psychiatric evaluation is not a test with a right or wrong answer. It is a structured conversation that helps a clinician understand a patient's history, symptoms, and the context around them.

It is also not therapy. The goal is to identify what is happening — whether that is ADHD, anxiety, depression, a mood disorder, trauma-related concerns, or something else — and build a clear path forward based on that finding.

At MHMATTERS365, evaluations are conducted by Vernisha Chambers Douglas, PMHNP-BC, or Jerrika Myles, PMHNP-BC — both board-certified psychiatric mental health nurse practitioners. The first appointment takes 60 to 90 minutes.

What to bring to the evaluation

The more information you can share, the more accurate the evaluation. Bring the following:

• A list of current medications, including dosage and how long the patient has been taking them

• Any previous psychological or neuropsychological testing reports

• School records, IEP documents, or 504 plans if the child has them

• Prior mental health treatment records — therapy notes, previous diagnoses, past medication trials

• Your own notes about what you have observed at home, and what teachers or others have reported

A referral is not required for most patients. The exception is Alabama Medicaid — Medicaid patients need a referral from their pediatrician or primary care provider before the first appointment. Submit the referral form here or fax it to (334) 249-2791.

What happens during the session

The session begins with history. The clinician asks about the patient's developmental history, family mental health history, current symptoms, academic performance, social relationships, and daily functioning. For child patients, parents are present for part or all of the session depending on the child's age.

The clinician is listening for patterns — how long symptoms have been present, in which settings they show up, whether they have gotten better or worse over time, and whether there is a clear trigger or the onset was gradual.

Ruling out other explanations is as important as confirming a diagnosis. Sleep disorders, anxiety, trauma responses, and learning differences all produce symptoms that look like ADHD. Getting that distinction right is what determines whether treatment actually works.

What happens after the evaluation

By the end of the first session, the clinician shares an initial clinical impression. A clear diagnosis is reached in many cases. For more complex presentations, the clinician will share a working diagnosis and explain what additional information or time is needed before confirming it.

If medication is part of the treatment plan, the clinician explains the options, the expected timeline for results, and what side effects to watch for. MHMATTERS365 does not prescribe controlled substances including stimulants. Treatment uses non-stimulant options and coordinates with therapists, schools, and primary care providers when that helps.

A follow-up appointment is scheduled before you leave. You go home with a plan, not just a diagnosis.
